FSSAI Document Checklist
Having your documents ready in advance speeds up the FSSAI application process significantly. Requirements vary slightly by license type.
For Basic Registration
- Passport-size photograph of the applicant
- Identity proof (Aadhaar Card / Voter ID)
- Address proof of business premises
- Self-declaration form (Form A)
- Business / firm details
For State License
- Form B, duly completed and signed
- Photo ID & address proof of proprietor/partners/directors
- Proof of possession of premises (rent agreement / ownership deed)
- List of food products to be manufactured or sold
- Food Safety Management Plan
- Layout plan of the processing unit
- Partnership deed / certificate of incorporation (if applicable)
- Water testing report (for manufacturing units)
For Central License
- Form B, duly completed and signed
- Certificate of Incorporation / partnership deed
- Photo ID and address proof of all directors/partners
- Layout plan of the processing unit
- List of directors with contact details
- Food Safety Management System plan
- Import Export Code (IEC), if applicable
- NOC from the local municipal body
All documents must be uploaded in PDF or JPEG format, each under 2MB, to avoid rejection during verification.
